Utmost effort is made to keep client information safe, secure, confidential, and up to date.

Information Collection

Client information is kept private unless a client consents to information being released to an identified person or organization through the completion of a release of information form.


  • Client information including legal and preferred name, address, phone number, insurance information if available, and photo identification is collected and necessary for a client to provide in order to establish services with OGTN.
  • Completed release of information forms are necessary if a client wants information shared with any other people or organizations.
  • Clients are permitted to rescind or end any past release of information forms previously signed.
  • This agreement is only changed in situations where the safety of a client or other people is a concern and the proper responders are notified.
  • In the event a staff member must make a disclosure without a client’s authorization, the client will be informed as soon as possible and when legally permissible.



Chart Access

Only staff involved in the client’s services are able to review a client’s chart.

Information Disclosure

OGTN may use or disclose your information to insurance companies, government agencies, or health plans to assist us in getting paid for our services. For example, information such as dates of treatment may be released to an insurance company in order to obtain payment.

Security Notifications

In the event of a security breach and client information is stolen by an unauthorized entity, the client will be notified as soon as possible.
